The insurance ID from your card and the Group number.
Your phone number, and email address the insurance has on file.
Your Date of Birth.
Once insurance is verified, we will contact you to set an appointment,.At this time, we are not accepting new Medicaid/Apple/State insurance patients.
We take the insurance, but are at maximum number of patients for those insurances.

For New Patients Making Your First Appointment – Welcome!

When you submit an appointment request, it is not confirmed until you receive a confirmation email.

Here’s how the process works:

  1. Schedule Online
    • Visit our website and select the appropriate visit type from the dropdown menu.
  2. Enter Accurate Information
    • Provide complete and correct demographic and insurance details to avoid delays in verifying your eligibility.
  3. Insurance Verification
    • It can take up to 3 business days to verify your insurance eligibility and confirm if we are in-network with your plan. Once verified, your appointment will be booked, and you will receive a confirmation email.
      New patients: Please include all insurance information in comments.
  4. Appointment Timing
    • Same-day or next-day appointments are not available for new patients.
  5. Establish Care
    • New patient appointments are focused on establishing care.
  6. Clinic Policy on New Patient Visits
    • Annual exams or elective procedures are not performed during a new patient visit unless there is a specific medical issue requiring immediate attention.
  7. Missed Appointments
    • If you schedule a new patient appointment and fail to appear or cancel without 24-hour notice, we will not be able to grant another new patient appointment.
